25 | Licensing Program Analyst (LPA), Shine Yu, conducted an unannounced Annual Inspection 08:40 am and was greeted and met with the Licensee, Guan Jian. Present are licensee, licensee's spouse, 1 helper and 10 children, including 3 under 2 years old.
LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC610), and Parents' Rights (PUB 393). Facility's operating days and hours are Monday to Friday 8:00 AM to 6:00 PM. LPA reviewed the current Children's roster. The last fire drill was conducted on 04/13/2026.
LPA reviewed a random selection of 6 children's files and observed that notification of parent's rights forms, immunization records forms, consents for emergency medical treatment forms, and Identification forms are in each file. LPA reviewed the current 15 minutes check log.
LPA also reviewed the staff files for the following records: Employee Rights (LIC9052), Statement Acknowledging Requirement to report Child Abuse (LIC9108), and Immunization Record showing immunity to measles (MMR), pertussis (Tdap), and a statement of declined influenza shot. Licensees and helper's pediatric 1st Aid and CPR certificates, expire on 01/19/2027. Licensee's mandated reporter training certificate expires on 05/08/2027.
Licensee accompanied with LPA to tour the facility. LPA observed the fire extinguisher (2A10BC, purchased on 05/02/26) by the front door in the entrance hall way. The smoke and carbon monoxide detector are in kitchen and family room. The off limits areas inside the home are bedroom 1,2, 3, 5, kitchen and adult bathroom. The children bathroom is clean and diapers are organized on the rack. There are age-appropriate toys in the family room and play yard for children. Licensee stated there is no firearm in the facility. There is no stairs in the facility.
