On this day Licensing Program Analysts (LPAs) Berumen, Elizabeth and Almaraz, Celi conducted a case management visit to clear deficiencies cited on 06/09/2022.
Plan of Correction (POC) due dates 06/14/2022 and 06/24/2022. Director sent the POC on the date/s requested, however there was a typo on the email address sent to elizabeth.berumen@dds.ca.gov; please note the address should read elizabeth.berumen@dss.ca.gov There were no children present during today's visit. Please note the correct hours of operation of the center: Morning program is 7:45 AM to 10:45 AM and afternoon program is 11:30 AM to 2:30 PM.
LPA obtained a direct phone number to the center (510)753-0774 and (408) 507-9992.

LPAs observed the following during today's inspection:
1. The flooring in the children's bathroom are clean.
2. The crawl space under that storage shed in the outdoor play area has been barricaded by a slab of wood.
3. LPA Almaraz observed the grass to be cut. Director spoke to Maintenance personnel (John Fong) who states the grass will be completely removed.
4. LPA Almaraz obtained a written plan of correction from Director, Yadira Rios stating she understands to comply with the conditions of the waiver. Primary facility file, Kidango Ohlone Chynoweth Center (facility 434404854) as the central administrative file for Criminal Record Association.
5. LPAs observed a new dishwasher.
6. LPAs observed a fire disaster drill was practiced on June 14, 2022.

Deficiencies cited were cleared as of this day. There were no deficiencies cited during todays visit.
