On 05/28/2025 at 11:35AM, Licensing Program Analyst (LPA) Jialing “Julianne” Zhu conducted an UNANNOUNCED COMPLAINT SITE INSPECTION. LPA met with Director Yaling “Eileen” Lin and Licensee Chuanhui (Daphne) Chao. LPA advised director of the nature of the inspection. A complaint with one (1) allegation regarding children’s Personal Rights was filed against the facility. Present during the inspection were 7 (seven) fingerprint-cleared staff and 48 children in care.
During the inspection, LPA conducted interviews with Director, Licensee, and two (2) staff. All staff interviewed gave statements that supported the allegation of staff handling a child roughly causing injury. LPA obtained copies of relevant documents relating to the incident from Licensee. Licensee also provided camera footage from date provided by the Reporting Party (RP) of an incident that supported the allegation. Based on LPA’s observations and interviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, 101223(a)(3), is cited on the attached LIC 9099D.
Notice of Site Visit was given and must remain posted for 30 days. Report was reviewed, exit interview was conducted, and Appeal Rights were provided with Director Eileen Lin.

The facility has already taken the following actions: an in-house investigation was completed and documented, staff who handled child roughly has been terminated, all-staff meeting was held to address how to safely provide care and redirect children.

Based on observation and interviews, the licensee did not comply with the section cited above as statements from interviews and camera footage supported the allegation of staff handling a child roughly, which poses an potential health, safety, and/or personal rights risk to persons in care.

LPA determined the actions taken by facility are sufficient to clear the citation. LPA recommended for director to review what the Personal Rights of children and Mandated Reporter requirements are with all staff members.
