On 07/07/2026, Licensing Program Analyst (LPA) Jaime Gonzales conducted an unannounced complaint investigation. LPA met with Site Director, Madlen Zayehmoureh and stated the reason for today's visit was to deliver investigation findings for the above allegations.

During today's investigation, LPA completed a census head count in the facility. The Jellyfish, Nemo and Sea Turtle children were all combined in the napping classroom. There were 20 preschoolers and two staff present.

It has been alleged that Staff (S1) yelled and handled C1 in a rough manner in the Nemo Classroom on April 1, 2026.

LPA conducted interviews with witnesses to the alleged incident and there was conflicting testimony about how S1 handled C1.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is UNSUBSTANTIATED.

No deficiency was cited during today's investigation.

Exit interview conducted and report was reviewed with the Site Director, Madlen Zayehmoureh. A notice of site visit was given and must remain posted for 30 days
