Licensing Program Analyst (LPA) Joe Macias conducted an unannounced Required - 1 Year Inspection (care tool). The purpose of today’s visit is to ensure the facility is in compliance with Title 22 California Code of Regulations. LPA met with the Administrator Shivashankari Velmurugan, and explained the nature of today's visit. LPA toured the Facility both inside and outside during todays visit. The facility also has an infant licence #434415770, on site in room #4. LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents' Rights Poster (PUB 393), Personal Rights (LIC 613A), Child Car Seat Law (PUB 269), and Activity Schedule. The hours of operation are Monday - Friday, 8am - 6pm. Room #3 (Bldg at 1686 Berryessa Rd) is currently not in use by the child care center. The Heritage program utilizes room #3, Monday - Friday, 1pm - 6pm. The overall capacity will decrease to 38 children whenever the Heritage program is in session. The facility has an active waiver for limited outdoor space. LPA Macias observed the waiver posted in the center.

Upon arrival to the facility in room #1 LPA Macias observed the center out of ratio, as well as out of compliance. The LPA witnessed a classroom Aide left alone with fourteen children. LPA Macias discussed the importance of maintaining the correct ratio. As well as reviewing the roles and allowances of an Aide. There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance. Explaining an Aide shall work only under the direct supervision of a teacher. A ratio of one fully qualified teacher and one aide for every 18 children in attendance in a preschool program is allowed when the aide meets the qualifications.


LPA reviewed ten children's and Five staff files during today's visit. Each child's file reviewed contains the Information and Emergency Information form (LIC 700), immunization records, physicians report, personal rights, and parents rights. All staff files contain the required transcripts/verification of experience. All staff's CPR and First Aid certifications are current. The Director understands that there shall be at least one person with valid CPR and First Aid certifications on site at all times, or present during off-site activities (field trips).

LPA observed all furniture and equipment is in good condition and safe for the children. The playground area utilized by children is surrounded by appropriate fencing. LPA observed that the outdoor equipment is age appropriate. LPA did not observe any bodies of water. The conditions of the waiver state the play yard capacity shall never exceed 35 children at any one time. There shall be a play yard schedule posted in the classroom. Children from the Heritage Program will not share with the playground use.

Director understands the conditions, limitations, and capacity specifications of the Facility license. Director understands that children shall be visually supervised at all times. LPA observed that all rooms are in order. The facility has not completed the required lead testing. Drinking water is readily available for the children in each room and in the outdoor playground area via water bottles brought from home, cups, and a drinking fountain. The LPA reviewed the importance of completing the required lead testing. LPA observed solid waste containers with tight-fitting lids in each room. Staff and children's bathrooms are clean, sanitary. There is a separate staff bathroom not utilized by the children which an isolated child can use if needed. Director states that there are no weapons or firearms on the premises.

The food preparation and storage areas are clean, free of litter & rubbish, and free of rodents and other vermin. Cleaning supplies are securely stored and inaccessible to the children. LPA observed a fully charged 2A10BC fire extinguisher, and working smoke/ carbon monoxide detectors. The last fire disaster drill was conducted on March 31, 2023. Director states that the Facility does not administer medications at this time. Facility does not provide transportation for children, however the Director understands that children cannot be left alone, unattended in parked vehicles.


The Administrator Shivashankari Velmurugan was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Type A & B deficiencies cited, exit interview conducted and report was reviewed with the Administrator Shivashankari Velmurugan. Appeal rights provided and reviewed. LPA Macias reviewed and provided a copy of the LIC9224, acknowledgement of receipt of licensing report.

A notice of site visit was given and must remain posted for 30 days.
