25 | Licensing Program Analyst (LPA) Ashley Lopez conducted a follow up visit for a Case Management Inspection that was conducted on 8/16/23, in response to alleged incidents that were self-reported by Director to Community Care Licensing (CCL) on 7/27/23. LPA met with Director Shivashankari Velmurugan, and explained the purpose of today's visit. LPA arrived during nap time, there were 15 children with 2 staff.
LPA had conducted interviews pertinent to this case management inspection during a previous visit, observed staff and children, and inspected the physical plant. LPA reviewed the incidents with the director and the action taken following the incident. LPA also reviewed staff and children's records on facility file. The incident involved two previous staff members (S1) and (S2); other staff members had witnessed S1’s and S2’s inappropriate interactions and lack of supervision at the facility. The director conducted and documented her own investigation and staff interviews. The director had submitted those documents to licensing when she submitted the LIC 624.
The director had spoken with S1 and S2 to address their lack of supervision and behavior with the children. Prior to today's visit, the director stated that S1 and S2 were let go from the facility. The Director explained the procedures set in place to ensure supervision is maintained one hundred percent of the time. All staff have been trained in the facility's supervision policy. The director stated that S1 and S2 were always accompanied by another teacher, the children were always supervised. The director stated she had communicated with all parents of the children in care regarding the incidents with S1 and S2. She stated they held a PTA meeting to inquire with parents about their concerns of the facility and staff. She stated that none of the parents had brought up any concerns about the facility or staff.
No deficiencies were cited during todays visit. Exit interview conducted and report was reviewed with the director, Shivashankari Velmurugan.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
