On Monday, October 25, 2021 at 10:30 am, Licensing Program Analyst (LPA) Manel Estoesta conducted an unannounced subsequent complaint investigation at the facility. LPA met with Director Helen Qiu and explained the purpose of today’s inspection. Present on this visit were 8 staff , 9 toddlers and 21 preschool children.
During the investigation the department completed a physical plant inspection, reviewed facility records and documentation, and conducted interviews. LPA interviewed the Reprting Party, parents, staff and preschool children determined there was not enough evidence to support the above allegations.
Based on the interviews and information obtained throughout the investigation, the above allegations are unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the above allegations are deemed UNSUBSTANTIATED.
The director was provided a copy of this report. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Helen Qiu.
