On 8/7/2024, at 12:24 PM, Licensing Program Analyst (LPA) Liridon Fici arrived unannounced to conduct a subsequent complaint investigation visit and to deliver findings on the above allegations. LPA met and was greeted by director, Hourieh Gharibi, and explained the purpose of today’s visit.

During the course of the investigation, LPA conducted record review, interviewed 7 staff, and obtained the following documents: Bright wheels roster for preschool, emails of children no longer attending the center, food menu (May 2024), children emergency contact information, admissions agreement, and staff roster.

It was alleged that; Staff are operating beyond the terms and conditions of the license and Staff did not apply sun block to daycare child...

Based on observation and interviews conducted with staff, 7 out of 7 staff mentioned that children are not being commingled between classes; staff stated there is enough staffing to cover another staff that is absent for the day. 7 out of 7 staff advised LPA during interviews that sun block is applied to children when their parents request sun block to be applied. LPA inspected children’s cubbies and observed sun block.

It was alleged that; Staff did not meet a daycare child's toileting needs by forgetting to put a pull up on a child during nap time resulting in wet clothing and betting, and Staff did not provide appropriate clothing for a daycare child. Based on interviews conducted with staff, 7 out of 7 staff stated children are taken to the bathroom before nap time to ensure children do not have accidents during nap time. LPA observed children being taken to the bathroom or changed by teachers and aids before nap time and during nap time. LPA confirmed with all staff interviewed that the center has extra clothing for children to wear if the child does not have extra clothes in their cubbies; staff mentioned that parents are notified to bring extra clothing for their child to wear. LPA observed extra clothing in children’s cubbies, along with diapers and lotion.

Based on interviews, record reviews, observations, and evidence gathered during the course of the investigation, it is concluded that although the allegations noted on this complaint may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur. The allegations are UNSUBSTANTIATED.

A notice of site visit was given and must remain posted for 30 days.
