On 02/25/2025 at 8:43am, Licensing Program Analyst (LPA) Farida Raja met with Directors, Josephine Dalit and Elisa Hernandez Vega and conducted an unannounced Case Management inspection in response to an unusual incident that occurred on February 11th, 2025 involving staff (S1) and preschool child (C1). Incident was self-reported by the facility to Licensing on February 12th, 2025.

LPA conducted a facility walk through. LPA interviewed Director and staff during today's inspection and obtained a copy of the children's roster. Based on interview with Director, Elisa Hernandez Vega she stated that Human Resources is conducting their investigation currently.


Based on available evidence and interviews, no deficiencies were cited today.

Exit interview conducted and report was reviewed with Director, Elisa Hernandez Vega.
