13 | Licensing Program Analyst (LPA) Martha Jimenez-Villanueva conducted an unannounced follow-up investigation today to deliver the investigation findings on the above-mentioned allegations. LPA met with Director Maria Atkins. LPA toured inside and outside the Day Care Center. Director Maria was providing care for two school aged children.
This Department reviewed documents and conducted interviews with relevant individuals. During the course of the investigation, LPA gathered inconsistent stories during interviews. Based on the available evidence, it is concluded that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
No deficiencies were cited today. Exit interview was conducted and the report was reviewed with the Director Maria Atkins in Spanish and Appeal Rights were handed to her.
A notice of site visit was printed and handed to the licensee, must be posted near the entrance to the home and must remain posted for 30 days.
