25 | Licensing Program Analysts (LPAs) Kassandra Medrano and Mel Matos met with David Walter & Doug Ikemoto, directors, for a case management inspection. LPAs explained the nature of today's inspection to David and Doug. The program is currently licensed in Rooms 5,6,7,8, and Library. The program previously requested to add two classrooms (Rooms F & 24 - capacity unchanged) to the existing license. LPAs conducted a previous case management inspection on 08/11/2022, and the following items were requested of the Facility:
1. Fire Clearance approval from the City of San Jose Fire Department.
2. Rental/lease agreement from the San Jose Unified School District verifying Control of Property for existing licensed rooms: Rooms 5, 6, 7, 8, & Library plus additional Rooms F & 24.
3. Capacity letter from the San Jose Unified School District for all rooms.
4. YMCA @ Booksin (430709472) has submitted a request to the Department requesting to drop Rooms F and 24 from their existing license.
LPAs conducted an exit interview with David Walter & Doug Ikemoto, directors, and advised them that the program has until November 10, 2022 to submit the remaining items listed below to complete the licensure of Rooms F & 24. LPAs reminded David and Doug that they will need to withdraw their request to add Rooms F & 24 to the license if they cannot submit the items listed below by November 10, 2022:
1. Rental/lease agreement from the San Jose Unified School District verifying Control of Property for existing licensed rooms: Rooms 5, 6, 7, 8, & Library plus additional Rooms F & 24.
2. Capacity letter from the San Jose Unified School District for all rooms.
No deficiencies issued. Notice of site visit issued and must remain posted for 30 days. |
