Licensing Program Analysts (LPA’s) Stephanie Collins, Susy Cervantes and Araceli Almaraz conducted a site inspection of the facility today and met with Director Chin-Yeuan Chang . LPA explained the purpose of the visit; The inspection is in response to facility’s request for an increase in capacity from 44 to 49 preschool children age range 2 to 6 years of age . A Fire Clearance was granted on 3/11/22 by SUNNYVALE FIRE PREVENTION. The facilities days and hours of operation are Monday to Friday, 8:30am to 6:00pm. Working phone number is 408 784-966. LPA’s observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents' Rights Poster (PUB 393), Personal Rights (LIC 613A), Child Car Seat Law (PUB 269), LPA also observed 7 staff and 44-day care children in the facility during today's inspection appropriate staff to ratio observed.

The facility did not construct additional space or make any structural changes to the physical plant; However, the facility has decided to utilize Room #6 and add its' square footage to the exiting square footage of the license to accommodate the requested increase in capacity. The last Facility Evaluation was conducted on 1/14/2022. LPAs toured the indoor and outdoor areas of the Facility with director John (Chin) during today's inspection. A functioning Carbon Monoxide detector and Fire Extinguisher (3A40BC) was observed. Director states that there were no changes to the facility layouts since the inspection on 11/07/2018 therefore, the prior indoor activity space was not measured during today's inspection only Room # 6.

TOTAL PRESCHOOL INDOOR SPACE: 1,812.03 sq. ft. divided by 35 = 51 children.

There are a total 4/60 toilets and 7/ 105 sinks available to preschool for children. Staff restrooms are located in the church building adjacent from the preschool. Sick children are using the staff restrooms to isolate when ill and isolation equipment is available.


The following outdoor measurement were taken 1/11/2018
Playground: (41.833x113.833 = 4761.976) + (63.667x40 = 2546.68) = 7308.656
7308.656 minus 138.125 (encumbered space) = 7170.531

TOTAL PRESCHOOL OUTDOOR SPACE: 7170.531 sq. ft. divided by 75 = 95 children.

The playground area is surrounded by appropriate fencing. There is resilient material under high climbing structures. Shade is provided by trees and building overhangs. Children bring their own water bottles during outdoor time. No bodies of water were observed during today’s inspection.

Incidental Medical Services (IMS) policy was discussed. Licensee/Director, John Chang stated that the facility has a IMS plan, but currently there are no children requiring IMS services. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Pictures and measurements were taken of Room #6, pictures were taken of staff restroom/proposed isolation area, licensure is pending and application will be under review by management.

No deficiency was cited . A notice of Site Visit was issued and is required to be posted near the entrance and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the Facility Representative, Chin-Yeuan CHANG. LPA advised Chin that a license for 49 Preschool children will be submitted to Licensing Management for the final review and pending approval.
