32 | LPA was informed that consent forms for photos and videos are signed by parents; LPA observed consent forms signed and dated by children of parents. Parents interviewed stated they do not have concerns regarding the care and supervision provided to their children. LPA was also informed that their children have not expressed behaviors indicating they do not want to attend the center.
It was alleged that staff do not meet the educational requirements to provide care and supervision to day care children and that staff are not following child’s dietary restriction. Based on interviews and observations conducted, LPA observed Staff 3 (S3) in the infant classroom with the Director and another aide on multiple occasions. S3 stated she is a floater and provides breaks on the preschool side but is not left alone with children and is accompanied by a qualified teacher. LPA inquired about dietary restrictions, and staff stated that a list of children’s dietary restrictions is posted on the preschool side near the whiteboard, where staff have access to review it. LPA observed the dietary restriction list for children in care on 5/7/2026.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is UNSUBSTANTIATED.
A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS.
