25 | On June 24, 2026, Licensing Program Analysts (LPAs) Marilou Monico and Tra Dinh met with Site Director, Alson Hsu, and Children's Pastor, Edward Lin, for an unannounced Case Management Inspection. The purpose of today’s inspection was to follow up on a self-reported incident submitted by the facility to the San Jose Regional Office on June 5, 2026, involving two children, C1 & C2. The alleged incident was regarding neglect/lack of supervision. An initial inspection was conducted on June 12, 2026. During today's inspection, LPAs toured the facility and a census was taken.
During the investigation, LPA interviewed staff, children, and C1’s parent, and obtained relevant documents. Interviews with staff and children indicated that children in Room 2 use the bathroom located outside the classroom near the playground. Only one child uses the bathroom at a time while the others wait on the bench or in line. Staff and children reported that a teacher is always present to supervise when a child uses the bathroom. Interviews further confirmed that children are supervised at all times.
Based on the information obtained, no Title 22 Regulations were violated.
Exit interview conducted and report was reviewed with Site Director, Alson Hsu. Appeal rights were provided.
Per Title 22, Division 12, of the California Code of Regulations, no deficiencies are cited.
This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
