1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 03/08/2024 at 8:45am, Licensing Program Analyst (LPA), Farida Raja, met with Director, Sherron Murphy-Brown, for an unannounced Required-3 Year inspection in the Infant Program. The facility is licensed to serve children ages birth to 24 months in seven (7) rooms. Facility has a preschool program (#434415794) located on the same premises. LPA observed the required posted materials in the front lobby. Days and hours of operation are Monday through Friday, 8:00 am to 6:00 pm. LPA toured the indoor and outdoor areas during today’s inspection and observed the following ratios: Infant 1- not in use at this time Infant 2- 3 teachers: 4 infants Infant 3- 1 teacher: 2 infants Toddler 1+ Toddler 2 (combined outdoors)- 6 teachers: 9 children Toddler 3- 2 teachers : 5 children Toddler 4- 3 teachers: 4 children Facility was in compliance with teacher to children ratio requirement. LPA and Director toured the indoor and outdoor areas of the Infant Program during today's inspection. There is a working telephone at the facility (408-990-8920). The classrooms, children's restrooms, staff restrooms, kitchen and outdoor area were inspected. LPA observed snack menus and daily schedules in each classroom. During today's inspection, LPA observed children eating or engaged in various activities under the visual supervision of teachers. Facility has central heating/cooling for comfort of children. Director states that there are no weapons including firearms on the premises. Disinfectants, cleaning products and medicines were stored inaccessible to the children in care. Furniture and equipment in the rooms inspected were age appropriate and in good condition, free of sharp, loose, or pointed parts. LPA observed appropriate changing tables that have raised sides and have washable vinyl changing pads that are at least 1 inch thick. Napping cribs and mats were appropriate. LPA observed pull out solid waste containers/ compost bins and pull out trash cans for the disposal of diapers in each room. Continued on Page 2

Restrooms and sinks for children were observed to be safe, sanitary, and in working condition. Staff has two separate bathrooms that can be utilized by sick children if needed.

LPA observed fire extinguishers, working smoke and carbon monoxide detectors, and fire pull stations. The facility has a built-in fire system. Log shows that the last Fire Drill was conducted on 01/30/2024 which is compliant with the six-month requirement for facilities.

Drinking water is readily available for children in each room and in the outdoor playground area via individual water bottles and refilled using filtered water pitchers. Facility is not subject to the Lead Testing requirement as it was constructed after January 1, 2010. LPA referred Director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

Facility provides AM and PM snack to children in care. Children bring their own lunches or order lunch through My Green Lunch catering service. Kitchen used to prepare snacks was observed to clean. Menu was observed to be posted. Foods and beverages were stored safely to prevent contamination and spoilage. Food storage and preparation areas were observed to be sanitary. All food and beverages that require refrigeration are stored in covered containers with an opening date written on top. Bottles and containers of food provided by infant's authorized representatives was observed to be labeled with infants name and current date and stored in the classroom refrigerator. LPA observed an allergy and dietary chart and a food preference chart posted in each classroom.

Two outdoor playgrounds, play equipment and sand box was inspected and observed to be fenced in and free of hazards. LPA observed play equipment was in good condition, age-appropriate, and has grass as resilient material to absorb falls. Areas around and under high climbing equipment and slides were cushioned with grass. Shaded rest area is provided by canopy and umbrellas. There were no bodies of water observed.

Children sign in and out procedures and logs were reviewed. Children's roster was reviewed. LPA reviewed six (6) infant and toddler children's files during today's inspection. Each child's file reviewed contained the required forms. Needs & Services Plans were on file. Facility was in compliance with the Safe Sleep regulations/requirements, including the Infant Sleeping Plan (LIC 9227) and 15 minute nap checks conducted through their My Bright Day app.

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Five (5) staff files were reviewed. All facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. All staff files reviewed contain the required transcripts/verification of experience and have current CPR and First Aid certification on file. All staff have the required immunization's against pertussis, measles and influenza (or statement declining the influenza vaccine). LPA also observed the required Mandated Reporter Training on file for all staff.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Director stated that facility does handle medications and currently has children in care who require Incidental Medical Services. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Medications at the facility were observed to be stored in the office area. Director stated that Epi-Pens are stored in each classroom in the emergency backpack and LPA observed that medication is stored out of reach of children. Director stated that any medications that require refrigeration are stored in the staff lounge. Isolation of sick child was discussed, and Director explained that sick children will be isolated in the director's office and isolation equipment will be provided. A staff restroom which is off limits to other children is used if necessary. Child is always under visual supervision while in care of the facility. For IMS information see PIN 22-02-CCP.

Director will provide an updated Emergency Disaster Plan (LIC 610) along with permission letters from relocation sites and a complete director packet for Director Sherron Murphy-Brown to LPA by 03/12/2024 as discussed during Required-3 year preschool program inspection.

Facility does not provide transportation to children. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
