25 | Licensing Program Analyst (LPA), Shine Yu, conducted an unannounced Annual Inspection and arrived at 1:57 pm. Licensee was absent for doctor appointment. Licensee called her employee Yang to open door for LPA. Present are 10 children who are taking nap time, employee Yang and 1 helper (S1).
LPA observed the following materials posted on the bulletin board, including the Facility License, Notification of Parents' Rights (PUB 393) and Fire Drill check log. The last fire drill conducted was 10/2//2024. Facility's operating hours are Monday to Friday 8:30AM to 6PM. All children enrolled are full time and LPA reviewed current Children's roster.
LPA reviewed 6 children's files and observed that notification of parent's rights forms, immunization records forms, consents for emergency medical treatment forms, and Identification forms are in each file. LPA also reviewed the staff files for the following records: LIC 9052 Employee Rights, the Immunization Record showing immunity to measles (MMR), pertussis (Tdap). Licensee's pediatric First Aid & CPR expires on Feb, 2025. Employee Yang's pediatric 1st Aid & CPR expires July,/2025. S1's background check is clear but not associate with the facility.
LPA toured the facility with Yang. Licensee keep the bathroom is organized and clean. There is smoke detector and carbon monoxide detector in family room and dining room. There are plenty of age appropriate toys for children both in living room and out door play area. Licensee stated there is no firearm in the facility. There is no fireplace. There is no body of water observed outside of play yard. The outdoor play area is enclosed with fence and safe for children. During the inspection, another employee (S2) arrived at 3:12 pm. Licensee arrived home at 3:27pm.
