25 | Licensing Program Analyst (LPA), Shine Yu, conducted an unannounced Annual Inspection 12:40 pm and was greeted and met with the Licensee, Yan Ying Liang.
LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC610), Parents' Rights (PUB 393) and Earthquake Preparedness Checklist (LIC9148). Facility's operating days and hours are Monday to Friday 8:30 AM to 6:00 PM. LPA observed a current Children's roster. The last fire drill was conducted on 10/8/2025. LPA stated the emergency drill has to perform once every 6 months. Present are licensee, 2 helpers and 10 children, including 5 under 2 years old. LPA stated the regulation 102416.5(d)(1).
LPA reviewed a random selection of 6 children's files and observed that notification of parent's rights forms, immunization records forms, consents for emergency medical treatment forms, and Identification forms are in each file. LPA stated that each child has to maintain the immunization record form.
LPA also reviewed the staff files for the following records: Employee Rights (LIC9052), Statement Acknowledging Requirement to report Child Abuse (LIC9108), and Immunization Record showing immunity to measles (MMR), pertussis (Tdap), and influenza. Licensee's pediatric 1st Aid and CPR certificate, expires 08/22/2027.
Licensee accompanied with LPA to tour the facility. The off limits areas inside the home are bedroom 1, 3, bathroom 1, kitchen and garage. LPA observed the fire extinguisher in the living room, the smoke and carbon monoxide detector are in family room and bedroom. The bathroom is clean and organized. There are age-appropriate toys in the living room, family room and play yard for children. Licensee stated there is no firearm in the facility. There is no stairs in the facility. There is no body of water observed outside of play yard. The outdoor play area is enclosed with appropriate fence and safe for children.
