On 07/29/2026 at 11:00am, Licensing Program Analyst (LPA) Jialing “Julianne” Zhu conducted an unannounced complaint site inspection. LPA met with Director Rachel Lim and Assistant Director Ashley Thomas. LPA advised director the purpose of the inspection is to deliver findings of the complaint investigation. Present during the inspection were 31 fingerprint-cleared staff and 105 children. Throughout the course of the investigation, LPA obtained copies of the current facility roster and personnel report. LPA also conducted interviews with Director, Assistant Director, and teacher involved in the incident. The areas of the facility where the incident had occurred was also toured. LPA determined the incident reported did occur at the facility. The incident was an isolated incident, and parents were notified appropriately. Assistant Director addressed the incident with staff during team meetings. Based on evidence that was available to LPA, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, 101223(a)(2) is being cited on the attached LIC 9099D. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted, report was reviewed, and Appeal Rights were provided to Director Rachel Lim.

Assistant Director had addressed the incident with the staff during team meetings. Director will send meeting minutes to LPA along with a signed written statement of actions taken or to be taken to ensure a similar incident will not occur at the facility.

Based on interviews, the licensee did not comply with the section cited above as staff used a hazardous product to clean daycare child in an isolated incident, which poses a potential health, safety, and/or personal rights risk to persons in care.
