13 | On 03/09/2026, Licensing Program Analyst (LPA) Mandeep Kaur conducted an unannounced follow up complaint investigation. LPA met with Licensee, Elana Rosenberg and explained the purpose of today's investigation: deliver investigation findings. Also, present were Licensee's Assistant(S1), Licensee's Spouse/assistant(A3) and 12 children(2 infants and 10 preschool age). LPA conducted complaint investigation comprising of observations, Licensee's interview, staff interviews, random parents interviews, and attempted interviews with children, records review and evidence gathered.
Based on interviews, on 02/02/2026, Licensee self admitted that an Uncleared staff(S2)was providing care and supervision to children in care since November 2025. A staff(S2)has been eligible and associated to the facility for the criminal background clearance as of 01/29/2026.

32 | Also, on 02/02/2026, Licensee self admitted that on 01/27/2026, a child was found unattended without an adult supervision on a step of the door of the home by a staff, outside of the day care home and staff did not provide adequate supervision to the day care child. Licensee did not report an Unusual incident report that occurred on 01/27/2026, in a timely manner to the Department. The Department received the Unusual Incident Report of the incident that occurred on 01/27/2026, on 02/04/2026. Based on LPAs records review, evidence gathered and interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED.
As a result of today's investigation, Type A deficiencies, Type B deficiency and total amount of $1000 civil penalties are being assessed today on attached documents. Appeal rights were provided.
Exit interview conducted and report was reviewed with Licensee, Elana Rosenberg.
A notice of site visit has been issued and must remain posted for 30 days.
Assembly bill 633 was provided and discussed with Licensee. LPA Mandeep Kaur informed licensee Elana Rosenberg that this report dated 03/09/2026 documents two Type A citations which shall be posted for 30 consecutive days as there are immediate risks to the health, safety, or personal rights of children in care.
Also, LPA Mandeep Kaur informed the licensee to provide a copy of this licensing report dated 03/09/2026 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. |
