Licensing Program Analysts (LPA) Oscar Huang conducted an announced pre-licensing inspection. LPAs met with Applicant representative, Han Lee and explained the reason for the inspection. The purpose of this inspection is Applicant is applying for an infant license. Applicant is applying for a capacity of 24 infants. The infant center is located on the 2586 Seaboard Avenue, San Jose, CA 95131 in Room 1. Applicant is also applying for an preschool center at the same location in Room 2, 3, 4 and 5. Han Lee is the owner of this applying infant program and is as well applying for the infant director position. LPA noted that Han is currently licensed a large FCCH with #434415693 at 1630 Peacock Avenue, Sunnyvale, CA 94087, and advised her that the large FCCH will be closed when the infant application is licensed.

There are areas to post required postings, such as notification of parent's rights, personal rights, daily activity schedule, sketch, car seat, and menu. There hours of operation are Monday through Friday 7:30AM to 6:30PM. The contact phone at the site is (408) 599-4381.

LPAs toured the inside and outside of the facility. Measurements were conducted during today's inspection.
The indoor measurements for the infant program is as followed:
Room 1: (36.833 x 28.833) minus encumbered space 127.994 = 934.011

Total space: 934.011 divided by 35 = 26 infants

The outdoor space is as followed:
(46 x 22.833) = 1050.316 divided by 75 = 14 infants.

Applicant submitted a waiver request for sharing the playground on a different time slot.

There are 2 tables, 12 chairs, 24 cubbies, 10 cribs, 14 napping mats, 5 potty chairs for the infant children. There is adequate equipment, supplies, and toys for the infant. LPA observed a changing table in the room that has raised sides (3 inches in height) and is within an arm's reach of a sink. LPA observed containers with tight-fitting lids for solid waste and diaper disposal in the room. First aid supplies are stored in the staff room and are inaccessible to children. Cleaning supplies are stored in the off-limits janitor closet located outside of the classroom.

LPA observed functioning smoke detector and a carbon monoxide detector in the room. The Facility has central heating/air conditioning. Facility will not be administering. LPA did not observe any fireplaces or wall heaters at the Facility. Han states that there are no firearms on the premises.

LPA observed that the facility is clean. Han understands that the Facility must be kept free of flies, other insects & rodents. LPA observed that all furniture and equipment are in good condition and safe for the children. Drinking water will be readily available for the children water dispenser and reusable water bottles.

The Facility will provide AM/PM snacks to the older infant children. Parents will be required to provide meals for the young infant children. Old infant lunch will be prepared and delivered to the facility by a licensed off-site caterer. There is an off-limits church kitchen for snacks preparation, located in the same building of the Facility. The kitchen is clean and equipped with hot and cold running water, sink, refrigerator, microwave, oven range, and storage space for food. Disposable utensils and plates will be utilized by the Facility.

The outdoor playground space is fenced. There is a fence to separate the preschool play yard and the infant play yard. Area around play structure have resilient material, and under the play structure are cushioned with material that absorbs falls. There is adequate equipment and toys for children outdoors. Drinking water will be provided outdoors via infants' own reusable water bottles, and refil water dispenser. There were no bodies of water observed. The Facility will not provide transportation. Shaded rest area is provided through pergolas and trees. LPA discussed with applicant that children cannot commingle when outside including when going to the yard and back inside.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. LPA reviewed with facility representative the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Only the applicant is associated to facility roster. Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA discussed the safe sleep regulations with applicant, Han and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed applicant, Han of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform.

Exit interview conducted and report was reviewed with Applicant, Han Lee.
LPA advised facility representative upon approval of Licensing Management, a license for an ifnat center will be granted and issued to Applicant and completion of the following:
- Applicant/director current CPR/First Aid certifications
- Install an additional hand washing sink
- obtaining a copy of granted fire clearance
