25 | Licensing Program Analyst (LPA) Kate Huang and Licensing Program Manager (LPM) Gladys Kuizon conducted an unannounced Case Management visit today. LPA and LPM met with Licensee Wang, Wei.
During today's visit, LPM and LPA reviewed records and interviewed staff. The licensee did not have a current children's roster. Licensee completed the children's roster during inspection. Children's dates of birth were inaccurate when verified against the children's records.
A ratio of 2 staff and 10 children were observed. 6 of 10 children present were under the age of 2.
As a result of todays inspection, one type A deficiency and one type B deficiency were cited, see LIC809-D.
LPA Kate Huang informed licensee that this report dated on 03/05/2025 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Kate Huang informed the licensee to provide a copy of this licensing report dated on 03/05/2025 that document one Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
