Regional Manager (RM) Jennifer Pare, Licensing Program Manager (LPM) Joel Segura, and Licensing Program Analyst (LPA) Samantha Yip met with Director Dinora Sanchez at the San Jose Regional Office for a schedule dInformal Meeting. The purpose of this Informal Meeting is to discuss the recent citations. The center was cited for teacher qualification, supervision, personal rights, criminal record clearance, limitation of license, and building and ground.

The citations are as followed:

101229(a)(1) Responsibility for Providing Care and Supervision:
On 07/18/2024, 07/25/2024, and 08/07/2024, the preschool program was cited for supervision. On 07/18/2024, LPA observed that a staff in the preschool program went to the adjacent room; leaving children with a therapist who is not fingerprint associated to the facility. On 07/25/2025, LPA observed that a staff in the preschool program went in the adjacent room; leaving children in the room. On 08/07/202, LPA observed that children were in the restroom by themselves. A staff walked over to the restroom.

101216.1(c)(1) Teacher Qualifications
On 08/07/2024, the preschool program did not have a qualified teacher in the room. There were staff whose transcript were from an international college that was not evaluated or was missing courses.

101216.3(a) Teacher-Child Ratio
On 07/18/2024, the preschool program was cited for being over ratio. LPA observed that there was one staff with 14 children and one staff with 16 children.

101223(a)(3) Personal Rights
On 07/18/2024 and 07/25/2024, LPA observed that a staff had grabbed a child by their arm. On 07/18/2024, a staff had a child sit down from 10:29AM to 10:37AM. Staff did not allow the child to get up when the child asked at the 5 minute mark.

101170(e)(2) Criminal Record Clearance.
On 07/18/2024, there was a staff who did not have cleared criminal record clearance and was caring for children.

101161(a) Limitation on Capacity
On 07/18/2024, the preschool program was cited for commingling with the school-age program. There is no waiver for the school-age and preschool program to commingle.

1012238(a) Building and Grounds
On 07/18/2024, the preschool program was cited for having equipment that was not in good repair. LPA also observed that there was mouse dropping in the outside activity area. Director stated that pest control comes twice a month to spray the building.

The facility submitted an updated staff schedule to Licensing to ensure that there is a qualified teacher and rooms are within ratio. Director stated that she has conducted staff meeting, written reminders, and one-on-one meetings with staff regarding supervision and personal rights. Director will also conduct observation in the rooms to ensure children are supervised at all times.

LPM discussed teacher qualification, ratio, criminal record clearance, personal rights, and supervision. A copy of the teacher qualification and ratio were provided to Director during today’s meeting. LPM discussed with Director that courses completed need to completed at an accredited or approved college or university. LPM also discussed with Director that children from different programs cannot commingle.

LPM Segura explained the informal meeting and the administrative process. Director was advised that continued non-compliance with Title 22 Regulations could result in their license being referred to CCL's legal department for review and possible action against the license. Assembly Bill 633 (Child Care Parent Notification Requirements) and Acknowledgement of Receipt of Licensing Reports (LIC9224) was also explained and provided to Director Dinora Sanchez.

Director understood that this department will increase monitoring of the facility for the next twelve months at the departments discretion.

Exit interview was conducted and report was reviewed with Director Dinora Sanchez.
