Licensing Program Analyst Jennifer “Jen” Beehler conducted an unannounced Complaint Investigation. Upon arrival, LPA was greeted by Director Dinora Sanchez and provided access to the facility. LPA stated the reason for the visit was to deliver findings.

On 04/15/2025, LPA opened an initial investigation inspection. LPA conducted interviews, file reviews, observed the facility, and collected relevant documentation. LPA returned to the facility on 4/29/2025, 6/18/2025 and 7/9/2025 throughout this investigation.

Based on confidential interviews and evidence gathered during the investigation process, the Agency has determined the above allegation is UNSUBSTANTIATED. This means although the allegation listed above may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.
