Licensing Program Analyst Jennifer “Jen” Beehler conducted an unannounced Complaint Investigation. Upon arrival, LPA was greeted by Director Dinora Sanchez and provided access to the facility. LPA stated the reason for the visit was to deliver findings.

On 04/15/2025, LPA opened an initial investigation inspection. LPA conducted interviews, file reviews, observed the facility, and collected relevant documentation. During that inspection, LPA observed an Aide alone in the preschool play yard with 15 preschool age children and no qualified preschool teacher present. On 04/29/2025, LPA observed one qualified preschool teacher with 13 children in a classroom.

Due to LPA observations, the Agency found that the preponderance of evidence standard has been met based on the evidence gathered and therefore the above allegation is found to be SUBSTANTIATED.

This deficiency has already been cited in previous reports dated 04/15/2025 and 04/29/2025. Facility has made the corrections necessary to address the non-compliance.

Due to today's inspection, no deficiencies are cited. Exit interview conducted with licensee Merat Ayalew, report was reviewed and provided along with appeal rights.
