Licensing Program Analyst (LPA) Jennifer “Jen” Beehler conducted an unannounced Complaint Investigation. Upon arrival, LPA was greeted by Licensee Merat Ayalew. LPA stated the reason for the visit.

On 07/15/2025, LPA conducted an unannounced Complaint Inspection to open the investigation. LPA observed the facility, reviewed files, conducted interviews and collected relevant documentation. On 07/22/2025, LPA conducted an unannounced Complaint Inspection. LPA conducted interviews, reviewed documentation and observed the facility.

During this investigation, LPA witnessed 16 out of 18 children stating they did not have an afternoon snack on 07/22/2025. LPA observed the facility provided apple slices without a grain approximately 15 minutes later which was not the snack listed for the facility menu.

Witness testimony confirmed Staff Person #1 (S1) and Staff Person #2 (S2) are transporting day-care children. LPA verified S1 and S2 do not have Mandated Reporter Training, are not CPR certified and S2 is not Criminal Background Checked. Based on witness testimony, it was revealed that the school age program uses the grassy outdoor area with picnic tables for classes. This space is not licensed or fenced in. LPA conducted interviews with the Licensee, Merat Ayalew on 06/18/2025. Licensee stated the facility did not use the white and maroon vans for transportation. 07/09/2025, LPA witnessed Director Dinora Sanchez return to the facility with (9) children in the white van. LPA interviewed the Licensee about the field trip that occurred on 07/09/2025 during the Complaint Investigation on 07/15/2025 and Licensee stated the remainder of the children were returned by the Director by completing multiple trips to the park to transport children. LPA observed the white van returning at 2:30 PM and never leaving the facility. LPA observed Director Sanchez present until she left the facility at 4PM.

Based on the evidence gathered during this investigation, the preponderance of evidence standard has been met. Therefore, the above allegations are SUBSTANTIATED. Investigation will continue to review deficiencies to be issued.

Exit interview conducted with Licensee Merat Ayalew. Report was reviewed and provided along with appeal rights.
