This report is a continuation of the complaint investigation conducted on 07/31/2025.

On August 18, 2025, Licensing Program Analyst (LPA) Jennifer “Jen” Beehler conducted an unannounced Complaint Investigation to conclude the investigation and deliver deficiencies relating to substantiated allegations.

On 07/15/2025, LPA conducted an unannounced inspection and observed the facility pantry with only two apples for the day’s food service. LPA was present during afternoon snack time and confirmed that 16 out of 18 children did not have snacks from home. Staff interview confirmed there was not enough snacks for the children in attendance. LPA observed that the posted menu showed that a ½ cup of juice and crackers would be provided that day. LPA observed only apple slices being made available.

Throughout this investigation, LPA has conducted interviews with licensee, Merat Ayalew. 06/18/2025, Licensee stated the facility does not offer transportation. LPA uncovered contradictory details through witness testimony, which revealed that the facility provides transportation for children and conducts regular “field trips” where children are transported to water parks, library and local parks. Licensee stated that the facility does not use the large white and maroon vans because they are in poor condition. On 07/09/2025, LPA observed the Director returning to the facility with 9 children and one other staff in the white van.

07/22/2025, LPA revealed through witness testimony that Staff Person #1 (S1) was providing transportation to the children and does not have Criminal Record Clearance. This is being cited on a separate report.

7/15/2025 & 07/22/2025, LPA revealed through interviews that the facility is conducting class on the picnic tables in the unlicensed grassy area that is not fenced in and is bordered by two parking lots. Based on interviews conducted, the facility allows the children to play soccer in this area every Friday.

Due to this investigation, deficiencies are cited, more details provided on the attached LIC9099. Exit interview conducted with Licensee Merat Ayalew, report was reviewed and provided along with appeal rights.
