Licensee also requested to increase her licensed capacity to 14 children. Licensee has been licensed as a small Family Child Care Home since December 20, 2021 at the same location, and is thus eligible to apply for the capacity increase. LPA met with Licensee Zhifeng Zhang and explained the nature of today's visit to her. LPA also observed licensee's spouse and 5 day care children (3 preschoolers and 2 infants, including her infant son) in the home during today's visit. Days and hours of operation are Monday - Friday from 8:00 AM to 6:00 PM. The adults that reside in the home are the Licensee and her husband. LPA advised Licensee that her infant son will be accounted to her capacity when present during her operation hours. A review of staff records on 04/13/2023 indicates that all Facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violate LPA observed Licensee has current CPR and First Aid certifications which are expiring 12/2023. LPA observed a current roster of the children and a fire and disaster drills log which is to be done at least once every six months. The last drills were conducted on 12/05/2022.

LPA reviewed all 5 children files. Immunization records are maintained and up-to-date, and updated in form PM 286. LPA observed Notification of Parents’ Rights is in each child’s file, Identification and Emergency Information, infant sleeping 15 min. check logs, and Individual Infant Sleeping Plan for infant under 1 year old for her own son. The facility carries the liability insurance under Farmers.

Licensee's Mandated Reporter Training is current and on file. The facility carries liability insurance under Farmers Insurance.

LPA observed no changes were made for the facility child care & off-limits area both the indoor and outdoor play areas since licensed. LPA observed an above fish pond (48 inches x 48 inches & 16 inches depth) in the back yard, surrounded with fence makes inaccessible to the children in care.

Supervision of children was discussed with Licensee, and she understands that she must be present in the home during day care hours and ensure that the children are supervised at all times. Licensee understands her capacity options. Licensee states that she does not transport children via vehicle but she understands that children cannot be left in parked vehicles unattended at any time.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

A fire clearance granted from the San Jose Fire Department was received on 4/11/2023: fire clearance granted for occ 14.

Based on today's inspection, LPA advised licensee that a Large Family Child Care Home license will be approved pending on the management approval:

No deficiency was cited. A notice of site visit was given and must remain posted for 30 days. The report was discussed and verbally translated into Chinese during the exit interview with Licensee, Zhifeng Zhang.
