25 | Licensing Program Analyst (LPA), Shine Yu, conducted an unannounced Annual Inspection 08:30 am. LPA was greeted with teacher and licensee ChiMei Wang.
LPA observed the following materials posted on the wall in the living room, including the Facility License and Emergency Disaster Plan (LIC 610). LPA stated Notification of Parents' Rights (PUB 393) needs to be posted. The last fire drill conducted was 11/07/2024. Facility's operating hours are Monday to Friday 8:30 AM to 6:00 PM. LPA acquired a photo of current Children's roster. Present are licensee, 1 employee and 10 children, all children are over 2 years old.
LPA reviewed a random selection of 6 children's files and observed that notification of parent's rights forms, immunization records forms, consents for emergency medical treatment forms, and Identification forms are in each file. LPA also reviewed staff file and observed employee right form and her Pediatric First Aid & CPR expires at 04/13/2026.
LPA also reviewed the licensee's pediatric First Aid & CPR expires at Nov/2025.
LPA toured the facility with licensee indoors and outdoors. The off limits areas inside the home, is kitchen, bedroom 2, 3, 4, garage, and the side living room that is on the left side of front entrance. The open areas includes main living room, bedroom 1 and bathroom. Licensee asked how to change off limits room to open room. LPA stated that licensee will need to submit new sketch floor plan to the Department and have Fire Department for safety inspection. There is smoke detector and carbon monoxide detector in the living room. The fireplace is barricaded and inaccessible to the children. LPA observed cleaning compounds and shampoo are under sink area in bathroom. Licensee removed those to higher place immediately. There are age appropriate toys for children. Licensee stated there is no firearm in the facility. There is no body of water observed outside of play yard. The outdoor play area is enclosed with appropriate fence and safe for children.
