Licensing Program Analyst (LPA) Mel Matos conducted a follow up prelicensing inspection to the Facility today. LPA met with Angie Loza, co-director, and explained the nature of today's inspection to her.

The initial prelicensing inspection was completed by LPAs Collins and Matos on June 23, 2022 and Tammy Lariz, Applicant representative/director was advised to submit the following items to the San Jose Child Care District Office:

1. Updated Application for a Child Care Center License (LIC 200A)
2. Updated fire inspection approval for 14 infant children

Angie states that the updated Application for a Child Care Center License (LIC 200A) was dropped off at the San Jose Child Care District Office on Tuesday June 28, 2022. Angie states that Tammy Lariz, Applicant representative/director, is on vacation and will be returning on Tuesday March 7, 2023.

Angie states that Tammy Lariz, Applicant representative/director, can drop off a "duplicate" copy of the Application for a Child Care Center License (LIC 200A) upon her return from vacation.

LPA advised Angie that the pending application is in the final review process and that the Facility will be notified once the license for 14 infant children has been approved.
