Licensing Program Analysts (LPAs) Dung Mac and Araceli Almaraz conducted an announced pre-licensing inspection to this facility today. LPAs met with Helena Geng, Applicant Representative, and Selene (Siqi) Lee, Site Director, and explained the nature of today's inspection. The Applicant has applied for a preschool (#434416365) and infant (#434416366) license on the same premises.

The Preschool Center will operate Monday through Friday from 8:00AM to 5:30PM, serving preschool children ages 2-6 years old. The Preschool program will be utilizing rooms 2 & 3 and the Infant program will be using room 1.

A fire clearance approval from the Santa Clara Fire Department was granted for 60 preschool children which was received prior to today's inspection. LPAs toured and measured the indoor and outdoor areas of the facility during today's inspection.

Indoor Activity Space:

ROOM 2:
Area A: (21.629 x 16.795 = 363.259) minus 60.536 (encumbered space) = 302.723
Area B: 21.676 x 14.770 = 320.155
Area C: (35.499 x 18.907 = 671.180) minus 128.798 (encumbered space) = 542.382

ROOM 3:
(28.707 x 37.736 = 1,083.287) minus 143.109 (encumbered space) = 940.178

TOTAL INDOOR SPACE = 2,081.478 sq. ft. divided by 35 = 60 children

Facility has 3 sinks (45) and 4 toilets (60) available for the children. The sinks used by children only have cold running water. Staff have a separate bathroom which is located in Room 3. Facility has a separate bathroom for sick children to use if needed. Sick children will be isolated at the director's desk and the isolation equipment will be provided. Staff and children's bathrooms are clean, sanitary, and operable.

Diaper changing will be provided. LPA observed a changing table (in Room 2) has raised sides (3 inches in height) and is within an arm's reach of a sink. LPA observed containers with tight-fitting lids for solid waste and diaper disposable. First aid supplies are stored in each classroom and inaccessible to children. Cleaning supplies will be stored on high shelves, in the off-limits staff bathroom. LPAs observed smoke/carbon monoxide detectors and appropriate lighting in each classroom. The facility has central heating/air conditioning. Medications will be locked in a lock box and will be stored in a locked cabinet at Director's desk.

LPAs observed that the facility is clean. Helena understands that the facility must be kept free of flies, other insects & rodents. LPAs observed that all furniture and equipment are in good condition and safe for the children. Drinking water will be readily available for the children indoors via filter water and drinking fountains. Transportation will not be provided by the facility.

The facility will serve AM/PM snacks and lunch to the children. There is an off-limits kitchen for food preparation. The kitchen is clean and equipped with hot and cold running water, refrigerator, rice cooker, and storage space for food. Disposable utensils and plates will be utilized by the facility.

There are 7 tables, 45 chairs, 60 cubbies, and 45 cots for the children. There is adequate equipment, supplies, and toys for the preschool children in each classroom.

OUTDOOR MEASUREMENTS:

(42.250 x 37.500) minus (3.583 x 8.083 - 4.500 x 10.000) = 1,510.414

TOTAL OUTDOOR SPACE = 1,510.414 sq. ft. divided by 75 = 20 children

Preschool playground has space for 20 children. There is not enough space for the requested capacity of 60. As a result, the facility will need to reduce the capacity.

LPAs observed the playground is fully fenced which meets the height requirements. Shade is provided by trees and building overhangs. The area under the play structure is cushioned with material (rubber foam) that will absorb a fall. Drinking water will be provided via pitchers and individual bottles. No bodies of water were observed. LPAs observed that the fence has the chipped wood and unsteady parts, the base of a tree does not have sufficient dirt to cover, the debris on the right side curtilage, the cracks on the left wall next to gate, and spider and cob webs on building overhangs.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. Applicant Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPAs reviewed with Applicant Representative the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Exit interview conducted and report was reviewed with Helena Geng, Applicant Representative. LPAs advised Helena that a license for 38 Preschool children will be submitted to Licensing Management for the final stage of approval upon receiving the following items:

1) Fence needs to be sanded.


2) The base of a tree needs to be filled in.
3) Debris on the right side curtilage needs to be removed.
4) Cracks on the left wall, next to gate, need to be repaired.
5) Spider and cob webs on building overhangs need to be removed.
6) Updated LIC 200A. Helena provided LPAs with an updated LIC 200A during today's inspection.
