On March 11, 2026, Licensing Program Analyst (LPA) Marilou Monico made an unannounced Complaint investigation. LPA met with Site Director, Lisa Alvarado, and explained to her the reason for the inspection. LPA toured the facility and census was taken.

LPA conducted staff interviews, reviewed records, and obtained copy of supporting documents. Based on interviews, on March 6, 2026, the children participated in a science activity on how to make slime. The children were divided into two groups: one group made the slime while the other group played with slime. During the activity, a child (C1) was playing with slime and accidentally got slime stuck in their bangs. Staff (S1) assisted the child in removing the slime from their hair using warm water; however, some slime remained. S1 then cut C1's hair to remove the remaining slime without obtaining prior permission from the parents. The preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.

On March 11, 2026, Licensing Program Analyst (LPA) Marilou Monico made an unannounced Complaint investigation. LPA met with Site Director, Lisa Alvarado, and explained to her the reason for the inspection. LPA toured the facility and census was taken.

Based on staff interviews and other available evidence, on 03/06/26, Staff (S2) informed C1's parent at pick up that Staff (S1) cut C1's hair. On February 18, 2026, C1's parent brought to their attention that C1 had a bite on the arm from another child (C2). Staff did not witness C1 sustaining a bite from C2. According to staff interviews, they observed C1 shove C2 earlier that day. However, none of the staff observed C1 crying, showing signs of distress, or touching their arm. The bite was located underneath C1's clothing and was not visible to staff during that day. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
