25 | On July 26, 2022 Licensing Program Manager (LPM) Mary Segura, and Licensing Program Analyst (LPA) Stephanie Collins met with licensee Manali Doshi in an informal in office meeting at the San Jose Regional Office located at 2580 North First Street Suit 300 San Jose CA. 95131 to discuss the licensee's application for increase of capacity on her Child Care Center license facility # 434416419 from 73- to 125 children and to discuss the most recent deficiency cited to the license on 12/01/21;Type A ”Responsibility for Providing Care and Supervision”- CCR Section 101229(a)(1)-(a)The licensee shall provide care and supervision as necessary to meet the children's needs - (1) No child(ren) shall be left without the supervision of a teacher at any time […] and 101230(c)(1). Licensee stated she never received the bill in the mail and to best of her knowledge, she was not aware. Supervision shall include visual observation. A civil penalty in the amount of $ 500.00 was accessed.
LPM also discussed with licensee the responsibility for providing care and supervision, Director’s responsibilities as well as the unpaid penalty accessed during the last case management on 12/01/2021. Licensee agreed and paid the assessment today (7/26/2022).
During the meeting the licensee had the opportunity to explain the reason (s) why she was out of compliance with the regulations. The status of her Plan of Correction “Supervision and Safety Plan”. She agreed submit the orientation for substitute staff training and submit to LPM. LPM explained to Manali the seriousness of the deficiency “Responsibility for Providing Care and Supervision” specially as it applies to providing childcare services to young preschool children.
Licensee was explained her options pertaining to the application submitted for an increase of capacity to her licenses, after the discussion of the facts, licensee today July 26, 2022 licensee decided to withdraw her application for an increase of capacity, so she can demonstrate to the Licensing Department that she can substantially comply with all the regulations governing the Child Care Center. Licensee was advised that the department will monitor the facility to ensure substantial compliance with the regulation prior to any increase of capacity being granted.
Licensee understands that in the event that she does not understands the regulations she will contact her LPA or the desk duty officer and will ask clarification on any given issue.

- Indoor and outdoor sketches high lighting Staffs Toilets and Sink, and Children Toilets and Sinks, outdoor activity area (Updated sketches requested).
- Submit staff orientation/protocols for substitutes staff training; including but limited to Care and Supervision".
