25 | Licensing Program Analyst (LPA) Mel Matos met with Applicants, Ramon Caceres Agelviz & Milly Rodriguez Agelvis, to conduct an announced prelicensing inspection at 1785 Ridgetree Way, San Jose, CA 95131. LPA was granted access to the home by both Applicants. LPA notes that the residence is currently licensed as a large Family Child Care Home (Zhayda Caceres Agelviz - #434413368) and five day care children were present during today's inspection. Current Licensee, Zhayda Caceres Agelviz will be closing out her license and moving out of the home as of Friday July 29, 2022. The Applicants and two adult daughters (Patricia & Alexandra Caceres Rodriguez) are the adults residing in the home. Current Licensee, Zhayda Caceres Agelviz will be closing out her license and moving out of the home as of Friday July 29, 2022. There are no minor children residing in the home.
Days and hours of operation will be Monday - Friday from 8:30 AM to 5:30 PM. Applicants have completed their Preventative Health and Safety Child Care Training on June 25, 2021 and copies of the certifications are on file. Applicants CPR and First Aid certifications are current and expire on July 3, 2023. Applicants completed the Mandated Reporter Training for Child Care Workers on July 21, 2022 and copies of the certifications are on file. A copy of current TB test, flu, Mmr, and Tdap vaccinations for both Applicants are on file. Applicants rent the home and a copy of the lease agreement verifying control of property and Owner/Landlord Notification (LIC 9151) & Owner/Landlord Consent (LIC 9149) forms are on file. Applicants do not have liability insurance at this time and will issue Affidavit Regarding Liability Insurance for Family Child Care Home (LIC 282) until they obtain liability insurance. An approved fire clearance from the San Jose Fire Department was received on August 30, 2021.
LPA toured the indoor and outdoor areas of the home with the Applicants during today's inspection. There are not stairs or wall heater units inside the home. Off limit areas in the home: master bedroom, master bathroom, one bedroom, furnace closet, locked bathroom vanity cabinet door, barricaded fireplace (located in the living room), and attached garage. Off limit areas outside the home: front yard and right side area of the backyard.
The home is clean, orderly, (including heating/air conditioning), and ventilation for safety & comfort. There is sufficient toys, supplies, and equipment for the day care children both indoors/outdoors. LPA observed a fully charged fire extinguisher (3A40BC) and working combo smoke and carbon monoxide detectors, fenced backyard, and no bodies of water. Applicants state that they do not have any weapons/ammunition in the home. |
