At 8:55 a.m., Licensing Program Analyst (LPA) Syeda Bahar and Licensing Program Manager (LPM) Belinda Devall met with Licensee, Eileen Advincula for an unannounced annual/random inspection. Present during the inspection were – The Licensee, Licensee’s spouse, (1) one adult assistant, (2) two infants (1) one preschooler, and licensee's son. Days and hours of operation are from Monday to Friday, 8:00 am to 6:00 p.m. The ages of the children to be served are from 0 months to 6 years. The LPA toured the indoor areas of the home to conduct a Health and Safety Inspection.

ON LIMITS AREA: Playroom, Living Room, Kitchen/Dining Room, and bathroom located on the second floor.
OFF LIMITS AREA: Entire third floor, office on the 1st floor and attached garage.

The home is clean and orderly, with heating/air conditioning, ventilation for safety and comfort. LPA observed a fully charged 2A10BC fire extinguisher, working smoke/carbon monoxide detectors and working telephone. The Licensee is reminded that the fire extinguisher must be service every year and proof of service must be readily available for review. The Licensee was reminded to conduct and document fire and disaster drill every six months. There are ample age-appropriate toys that appear to be safe and in good condition. The bathroom is observed to be clean, sanitary, and operable. There are no bodies of water observed. Cleaning products, disinfectants, sharp objects were stored and inaccessible to children. Furniture and equipment were observed to be age appropriate and in good condition. For outdoor play, the Licensee takes the children to a gated park.

Licensee has a valid CPR and First Aid, completed on 09/15/2024 (expires September 2026). Licensee completed Mandated Reporter training on 09/23/2021. One adult assistant does not have proof of completion for mandated reporter training. Licensee was reminded of Mandated Reporter Training (AB1207) and CPR, First aid certifications needs to be renewed every two years. Licensee was reminded that all immunization records are needed to be available for review and on file. Per Licensee, there are no firearms in the home. Child’s records are available for review. Children files do not contain all required forms. Licensee does not have all REQUIRED forms posted and visible for public review.

There were no baby walkers or bouncers observed in the on-limit areas for the children. Licensee understands that baby walkers or other similar items are not permitted for the children. All cribs or play yards meet the safety standards. The cribs or play yards do not hinder entrance or exit to and from the space where infants are sleeping. The mattresses in the crib or play yards are firm and covered with a fitted sheet that is appropriate, fits tightly, and overlaps the underside of the mattresses. Each infant’s bedding is used for them only and cleaned weekly or before use by another infant. There are no loose articles and objects, bumper guards or objects hanging above or attached to the side of the cribs or play yards.

Licensee does not have Individual Infant Sleep plan and the 15 minutes check infant sleep log for any infant on file. Licensee was reminded that each infant under 12 months of age must have an Individual Infant Sleeping Plan maintained in the file signed and dated by the infant’s authorized representative, including the infant’s sleeping position and documentation of 15 minutes checks while sleeping.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Forms of discipline to be used by Licensee are redirecting and talking with the child. Licensee understands that children's personal rights should not be violated, including but not limited to, no corporal punishment, children are treated with dignity, receive safe, healthful, and comfortable accommodations, interference with eating, intimidation, or other actions of a punitive nature.

LPA observed that one (1) adult assistant does not have completed the background check and finger print clearance. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/resources/child-care-centers/. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Licensee, Eileen Advincula, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Megan's Law was checked on 02/06/2025. To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. During today’s inspection, (1) one "Type A" and (6) six “Type B” deficiencies are issued on attached 809-D. Appeal rights provided. LPA Syeda Bahar informed licensee, that this report dated 2/10/2025 shall be posted for 30 consecutive days as there are immediate and potential risks to the health, safety, or personal rights of children in care.

Also, LPA Syeda Bahar, informed the licensee, Eileen Advincula to provide a copy of this licensing report dated 02/10/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Licensee, Eileen Advincula.
