25 | Licensing Program Analyst (LPA) Mel Matos met with Cindy Traidman, Licensee, for an unannounced Required – 3 year annual inspection. LPA was granted access to the home by the Licensee's assistant, Chaoyu Kawamura. Licensee arrived at the home approximately 10 minutes after LPA's arrival. LPA also observed eight day care children (2 infant and 6 preschool) and three adult assistants (Chaoyu Kawamura, Anna Dovhan, & Angie Chen) in the home during today's inspection. Licensee was operating within the capacity and ratio requirements of her license. LPA observed the required postings, including the facility license, near the main entrance to the home. Days and hours of operation are Monday - Friday from 8:30 AM to 6:00 PM. Licensee is the only adult residing in the home. Licensee has one minor daughter, age 16 years, residing in the home.
LPA reviewed a current Child Care Facility Roster and Fire/Disaster drill log during today's inspection. The last fire/disaster drill was completed on December 15, 2023. Licensee states that she does have liability insurance for the day care through DCI Insurance (expiration: 03/2024). Licensee has current CPR and First Aid certifications (expiration: 08/26/2025). Licensee also has two staff who have current CPR/First Aid training certifications on file. Licensee and her staff have the required vaccines (MMR, Tdap, & flu - opt out) and are current with their Mandated Reporter Training for Child Care Workers. LPA reviewed six children's files and the files were complete with the required forms and immunization records. LPAs reviewed four staff files (Licensee & 3 adult assistants) and the files were complete with the required forms and immunization records.
LPA toured the indoor and outdoor areas of the home during today's inspection. Licensee has a working telephone in the home (408-393-7565). The home is clean, orderly, (including proper heating/cooling/ventilation), and safe for the day care children. There are safe & age appropriate toys, play equipment, and materials for the children in the home. There are no stairs and no wall heater/fireplace units inside the home. The off limit areas inside the home: one bedroom, two bathrooms, laundry room, and attached garage. The off limit area outside the home: back and left side areas of the backyard, barricaded air conditioner unit, and locked storage shed (located in the left side area of the backyard).
LPA observed a fully charged 3A40BC fire extinguisher, working smoke/carbon monoxide detectors, no bodies of water, and fenced backyard. Licensee states that she does not have any weapons or pets in the home. All detergents, cleaning compounds, medications, and other similar items are inaccessible to children. Any poisons are stored in the off limits storage shed. |
