25 | Licensing Program Analyst (LPA) Oscar Huang conducted an unannounced annual and a capacity increase request visit to the facility today. Licensee has been licensed as a small Family Child Care Home since February 24, 2022 at the same location, and is thus eligible to apply for the capacity increase. LPA met with Licensee Yu-Bao Wu and explained the nature of today's visit to her. LPA also observed a helper and 6 day care preschoolers in the home during today's visit. Days and hours of operation are Monday - Friday from 8:30 AM to 6:00 PM. The adults that reside in the home are the Licensee and her husband.
A review of staff records on 03/02/2022 indicates that all Facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violate
LPA observed Licensee has current CPR and First Aid certifications which are expiring 11/2023. LPA observed a current roster of the children and a fire and disaster drills log which is to be done at least once every six months. The last drills were conducted on 02/15/2023. LPA reviewed 6 children files & the helper's file. Immunization records are maintained and up-to-date, and updated in form PM 286. LPA observed Notification of Parents’ Rights is in each child’s file, and an infant 15 min. sleeping check logs under 2 years old prior to turn into 2 years old. |
