25 | Licensing Program Analyst (LPA) Mel Matos met with Michelle Berkman, Licensee, for an unannounced Annual/Random inspection. LPA was granted access to the home by the Licensee. LPA also observed Licensee's spouse and two preschool children in the home during today's inspection. Licensee was operating within her capacity and ratio requirements. LPA observed the required postings, including the facility license, near the front entrance to the home. Days and hours of operation are Monday - Friday from 8:30 AM to 5:30 PM. The adults residing in the home are: Licensee and Licensee's spouse (James Berkman). There are no minor children residing in the home.
LPA reviewed a current Child Care Facility Roster and Fire/Disaster drill log during today's inspection. The last fire/disaster drill was completed on November 20, 2024. Licensee states that she does have liability insurance for the day care through Hays Companies, Inc Insurance and LPA observed a valid policy verifying coverage (exp: 09/07/2025). Licensee has current CPR and First Aid certifications (expiration: July 2025). Licensee has the required vaccines (MMR, Tdap, & flu - opt out) and is current with her Mandated Reporter Training for Child Care Workers (expiration: 05/15/2026). LPA reviewed three children's files and the files were complete with the required forms. LPA reviewed one staff file (Licensee) and the file was complete with the required forms.
LPA toured the indoor and outdoor areas of the home with the Licensee during today's inspection. Licensee has a working telephone in the home (#510-613-5825). The home is clean, orderly, (including heating/air conditioning/ventilation), and safe for the day care children. There are safe & age appropriate toys, play equipment, and materials for the children in the home. There are no wall heater units inside the home. LPA notes that the home is two story and the downstairs area of the home is used for the day care. There are stairs inside the home that are properly barricaded. Off limit area inside the home (1st floor): barricaded fireplace unit, 2 locked cabinet doors (under kitchen sink), attached garage, & laundry room. Off limit areas inside the home (entire 2nd floor): primary bedroom, office #1 & #2, sitting room, and two bathrooms. Off limit areas outside the home: front yard, barricaded air conditioner unit, and left side section of the backyard.
LPA observed a fully charged 2A10BC fire extinguisher, working smoke/carbon monoxide detectors, no bodies of water, and fenced backyard. The Licensee states that she does not have any weapons in the home. Licensee states that she has three cats in the home and she understands that any pet food/treats/litter boxes must be inaccessible to children. All detergents, cleaning compounds, medications, and other similar items are inaccessible to children. Licensee states that there are no poisons in the home. |
