25 | On 12/16/25, Licensing Program Analyst (LPA), Syeda Bahar met with Licensee, Anh Dao Nguyen, to conduct an unannounced annual inspection for an increase in capacity initiated by Licensee. The purpose of today's inspection: address the Application for a Large Family Child Care Home License (Capacity Change) that the Licensee submitted to the Department on 11/6/2025. Licensee is currently licensed for Small Family Child Care Home (FCCH). Licensee has been licensed from 12/5/2024 under license #434416586; therefore, she has at least one year of experience as a family child care provider to apply for a Large FCCH. A fire clearance for Large Family Child Care Home was granted on 12/1/25. Present for this inspection were Licensee, licensee’s husband, son and one assistant, five (5) children and (1) infant (19 months old). Licensee, her husband, son are the three adults living in the home. Licensee’s thirteen year old daughter also lives in the home.
The home is Single Family Home. The home consists of three bedrooms, two bathrooms, Livingroom, kitchen, driveway/front yard, backyard/play area, garden, side yard, and garage. The home was toured to conduct a Health and Safety Inspection. Days and hours of operation are from Monday to Friday 8:00 AM to 6:00 PM. The ages of the children to be served are from 3 months infant to 5 ears old. LPA toured the indoor and outdoor areas during today's visit. On-limit areas: Living room/play area, eating area, one bathroom and backyard/outdoor play area (right side of the yard near the garage). Off-limit areas: Three bedrooms, one bathroom, kitchen, garage, driveway, left side of the backyard, and garden. The Isolation area: Eating area near the kitchen. Licensee was advised to repair the safety screen/gates on the kitchen area to keep the off-limit area separate from the living room. There are ample age-appropriate toys that appear to be safe and in good condition. The backyard is fenced and there are no bodies of water. Licensee was reminded that all hazardous materials and toxins must be kept out of the reach of children.
