25 | On 2/2/24, at 1:33 PM, Licensing Program Analyst (LPA) Doni Fici arrived unannounced to conduct a Required 3-Year inspection and was greeted by Licensee, Medina, Macyet, and explained the purpose of the visit.
Days and hours of operation are Monday to Thursday from 6:30 AM to 6:30 PM. Licensee stated that, the licensee, her husband are the adults residing in the home. Licensee has four (4) kids that are under 18 years old of age that live in the home with the licensee. Licensee's certifications for CPR and First Aid is current
LPA toured the indoor and outside areas of the home with Licensee during today's inspection. LPA reviewed one (1) child's file and observed that parent's rights forms, immunization records forms, consents for emergency medical treatment forms, and Identification and emergency information forms are in each file. LPA review one (1) staff file and observed all forms/documentation's. LPA observed that last fire drill was documented on 10/2023.
The Licensee has a working cell phone in the home. LPA observed sufficient materials, toys, and play equipment for the day care children. LPA observed there are not stairs in the home. The off-limit areas of the house is the entire upstairs, kitchen, living room, licensees sons room, and laundry room/garage. All cleaning supplies, disinfectant, and other items that are dangerous to children were inaccessible to children.
LPA observed a fully charged 2A10BC fire extinguisher serviced 2/2023, with working smoke detector that are interconnected with the carbon monoxide detectors. The Licensee states that she does not have any weapons in the home. All detergents, cleaning compounds, medications, and other similar items are stored inaccessible to children.
