25 | On July 17, 2023 at 8:57 AM, Licensing Program Analyst (LPA) Marilou Monico conducted an announced Prelicensing Inspection in response to a change of location and change of capacity application. LPA notes that the Applicant is currently licensed at 871 Lily Avenue, Cupertino, CA 95014 (Facility number: 434416701). LPA met with Applicant, Ale Kaltenis, and explained the purpose of today's visit. Also present in the home was Applicant's husband. Per Applicant, the adults that reside in the home are herself, her husband, and her stepson. The home is a two story with 5 bedrooms and 4 bathrooms.
Days and hours of operation will be Monday - Friday from 7:30 AM to 9:00 PM. Applicant has completed the Preventative Health and Safety Child Care Training including Nutrition and Lead Poisoning Prevention and a copy of the certifications are on file. Applicant's CPR and First Aid certifications are current and expire on April 10, 2025. Applicant completed the Mandated Reporter Training on December 11, 2021. Applicant's immunizations in measles, pertussis, and flu are on file. Applicant states that she is planning to obtain daycare insurance once licensed.
The Applicant provided proof of control of property. Because the Applicant leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC 9151) that the applicant confirms was provided to the property owner/landlord.
LPA toured the indoor and outdoor areas of the home during today's inspection. LPA observed gated kitchen, gated stairs, glass covered fireplace, fully charged 3A40BC fire extinguisher, functioning smoke and carbon monoxide detectors. Off limit areas inside the home are: entire upstairs (Bedrooms 3 thru 5 & 2 bathrooms), kitchen, laundry room, office, and one closet on the first floor . The home is clean and orderly, with heating/air conditioning, and ventilation for safety and comfort. There is sufficient toys, supplies, and equipment for the day care children. Applicant states that there are no weapons in the home. Off limit areas outside the home: garage, storage shed, and chicken coop area. LPA observed a zipline in the backyard. LPA advised Applicant to follow manufacturer's recommendations including usage, safety, supervision, and ages of children who can use the zipline. LPA observed two dogs (Shih Tzu) and four cats in the home. Applicant states that the dogs and the cats have up-to-date shots. There were no bodies of water observed.
