Licensing Program Analyst (LPA), Farida Raja and Licensing Program Manager (LPM), Gladys Kuizon conducted an unannounced complaint investigation of the above allegations. LPM and LPA met with Director of Operations, Mylynh Huynh and Owner, Sima Shah.

On February 10, 2023, the department received a complaint alleging that an unqualified staff is allowed to supervise children alone and staff do not adequately supervise children resulting in children wandering to other parts of the facility without their teachers' knowledge.

A review of facility records was conducted. Staff roster, schedule, and training records were reviewed. Based on seven random staff records reviewed, all staff have the required qualifications for their job responsibilities.

Staff interviews were conducted. 7 out of 7 staff who were interviewed stated that they did not witness any child left unsupervised/unattended at any time. All teacher aides and support teachers stated that they are never left alone with children without the presence of a fully qualified teacher. All seven staff interviewed stated they have never observed children left alone with staff that is not fully qualified.

The department investigated the above allegations. Based on LPM and LPA observations, interviews conducted and records reviewed the Department found that the above allegations are UNSUBSTANTIATED.

An unsubstantiated finding indicates that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the above allegations did or did not occur.

Exit interview conducted with Director of Operations, Mylynh Huynh and Owner Sima Shah.

A Notice of Site Visit was issued and must remain posted for 30 days.
