LPAs met with Applicant Weidong Li and Director Marcia Tai, explained the reason for the inspection. The purpose of this inspection is Applicant is applying for change of ownership (preschool license with the toddler option). Facility is currently licensed as Appleseed Montessori School (facility #: 434413685). Applicant is applying for a capacity of 168 preschool children and 20 for the toddler option. There are areas to post required postings, such as notification of parent's rights, personal rights, daily activity schedule, sketch, car seat, and menus. There hours of operation are Monday through Friday 7AM to 6PM. There is working phone at the site, which is 408-507-0032. The following are indoor measurements and information: Preschool Program The Indoor measurements contain a total of {5634.82} square feet, which will not accommodate Applicant's request for 168. Total indoor measurements can accommodate a total preschool capacity of 160. Outdoor total measurements contain a total of { 12756.74 } square feet, which will accommodate Applicant’s requested capacity. Toddler Program The Indoor total measurements for blue room in building 1 contain a total of {750.68} square feet, which will accommodate Applicant’s request for 20 toddlers. Toddler Yard: Outdoor total measurements contain a total of {1509.18} square feet, which will accommodate Applicant’s request for 20 toddlers .

There are 208 chairs, 27 tables, 208 cubbies, 195 cots, 13 toilets, and 13 sinks for the preschool program. There are toys and equipment for children, which are age-appropriate. LPAs reminded that all disinfectant, cleaning supplies, or other items that could pose a risk to children need to be inaccessible. LPAs also discussed that minimum temperature in the room, which is 68 degrees Fahrenheit. There is fully charged fire extinguisher, smoke detector, and carbon monoxide detector. There is a first aid kit, which has all the required items.

For the toddler option, Building 1 Blue room there are 20 chairs, 3 tables, 24 cubbies, 20 cots, 2 toilets, 1 potty chair, and 1 sinks. There are toys and equipment for children. There a partition that separates the room, which is at least 4 feet high. There is a changing table, which is within arms reach of a sink. Changing table has a padding at least one inch and a raised side that is at least 3 inches. There a trash can that has a tight fitted lid. Isolation area is in the staff office and they will use one of the toilets and sinks in the toddler room.

The outdoor space is fenced. There is a fence to separate the toddler yard and the preschool yard. There is a play structure in the preschool yard. Area around play structure have resilient material. There are toys and equipment for children. Shaded rest area is provided through canopy and trees. LPAs discussed with Program Directors that children cannot commingle when outside including when going to the yard and back inside.

All meals are provided by parents. Facility will provide 2 snacks. Drinking water is provided through water bottles. Trash can for waste have tight fitted lids. LPAs discussed with trash can for waste need to have a tight fitted lid.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. LPAs discussed the safe sleep regulations with Site Director and discussed the Child Care Licensing Safe Sleep webpage athttps://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA/LPM informed Site Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPAs reviewed with Site Director the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
