On 7/16/2025, Licensing Program Analyst (LPA) Andy Yang conducted an unannounced complaint investigation. LPA met with Director, Selena Altman to deliver the complaint allegation of the facility operating out of ratio. Present for today's inspection were Director, (6) Children (Preschool age) , and (2) Staff. LPA toured the indoor areas of the facility.

During the complaint investigation, LPA reviewed children’s files, staff files, sign in and out sheet for staff and children. LPA interviewed the owner, director, teachers and parents. Through record reviews, observations, and interviews, there was no indication of the facility operating out of ratio.

On 12:15 PM, Licensing Program Analyst (LPA) Andy Yang conducted an unannounced complaint investigation. LPA met with Director, Selena Altman to deliver the complaint allegation of staff are comingling children of different age groups in the classroom. Present for today's inspection were Licensee, (2) Staff, and 13 Children (Preschool age). LPA toured the indoor areas of the facility.

During the complaint investigation, LPA reviewed staff and children's file, and interviewed the owner, director, teachers, and parents. On 6/27/2025, LPA Yang had observed during drop off time around 8:45 AM in the preschool room, (1) infant, (6) toddlers, and (4) preschool age in the same room. There was evidence of children being commingled during pick up and drop off time. Therefore, the facility was operating beyond the conditions specified on the license.

Based on LPA’s observation and interviews which were conducted and records reviewed, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED California Code of Regulations, Title 22 101161(a), is attached on the LIC 9099D.

One (1) Type B deficiency issued during today’s inspection.

· Operating beyond the conditions of the license.

A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted, report was reviewed, and Appeal Rights were provided to Director Selena Altman.
