Licensing Program Analyst (LPA), Farida Raja, conducted an unannounced complaint investigation to deliver complaint findings. LPA met with Director, Eun Jin Jeun and informed her of the purpose of the visit.

LPA toured the facility and observed ratios during today’s inspection. During the course of this investigation, LPA toured the facility, interviewed owner, director, staff, and parents and reviewed relevant records.

The Department received complaint allegations due to child, C1 leaving their infant/toddler classroom and teachers, during transition from outdoors to indoors, and joining the preschool class in the outdoor area. Based on director and staff interviews, infant/toddler and preschool staff failed to observe child leaving their group and joining the other. Based on record review and staff interviews, staff did not complete transition protocols requiring them to count children and conduct name to face checks resulting in child not being accounted for.

LPA also obtained varying accounts of where child was found and who escorted child back to the classroom.

Based on evidence gathered at this time the Department found that the preponderance of evidence standard has been met and the above allegation of lack of supervision is found to be SUBSTANTIATED.

One Type B deficiency was cited today as a result of the complaint investigation. Appeal Rights were provided to Director. Civil penalty issued.

Exit interview was conducted and report and plans of corrections were reviewed with Director, Eun Jin Jeun and Owner, Miao Li.

A NOTICE OF SITE VISIT WAS ISSUED AND MUST REMAIN POSTED FOR 30 CONSECUTIVE DAYS.

Director to submit a written plan of correction including training that was provided to staff regarding transition protocols and supervision of children. Director to include how staff will prevent commingling of infants/toddlers with preschool children in the outdoor area during transitions. Plan of correction due date 09/12/2025.

Teachers gave LPA varying accounts of where child was found and who escorted child back to their classroom. This poses a potential risk to the health, safety and personal rights of children in care.
