On 10/15/2025, Licensing Program Analyst (LPA), Farida Raja conducted an unannounced complaint visit to deliver investigation findings for the above allegations. LPA met with Director, Amparo Rios and explained the purpose of today's visit.

During today's inspection, LPA toured the facility and observed ratios. LPA observed a total of children 28 children and 12 staff within the five classrooms toured at the facility. Children continued to arrive during today's inspection. Facility is operating within ratio requirements.

During the course of this investigation, LPA interviewed director, staff and parents and reviewed relevant records. Based on director and staff interviews, they stated that there were several children, approximately 3 or 4 children, that were observed with bug bites during the summer months. Director stated that when the facility was notified, they started pest control services immediately to prevent any additional incidents. LPA obtained the service agreement and invoice of the pest control services for verification. Director stated that they have continuing monthly services with the pest control agency.

Based on LPA observations, children’s classrooms were observed to be clean, and LPA did not observe any bugs or infestations. Based on staff interviews, all staff that were interviewed stated that they have not obtained any bug or insect bites while at this facility and did not observe any child getting bitten while at the facility after the initial incidents. Staff stated that they started conducting health checks and would inform parents through the app. if they notice any bug bites on children. Staff stated that they sanitize and clean the facility throughout the day and at the end of the day they vacuum, mop, wipe with Lysol wipes, use bleach spray on toys and take the trash out. Bedding is placed in cubbies and taken home to be cleaned every Friday. Parent’s that were interviewed stated that they have observed the facility to be clean and their children have not come with any insect or bug bites from this facility.

Staff stated that if they are informed at the time of enrollment that a child is toilet trained then they do not help the child. If parents communicate with them and provide consent, then staff will help wipe the child. They also help a toilet trained child if child asks for help by providing directions. Staff stated that they always help children with hand washing. Staff stated that if a child is toilet trained then staff wait outside behind the barrier (ledge) and will provide assistance if requested. They stated that no children are allowed to go to the bathroom without staff.

Based on parent interviews, parents stated that they have not seen any children go alone or unsupervised to the bathroom and they feel that their children are adequately assisted in the bathroom.

Based on evidence gathered at this time for the above allegations, it is concluded that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. The allegations are thus UNSUBSTANTIATED.

No deficiencies were cited. Exit interview conducted and report was reviewed with Director, Amparo Rios. Appeal rights provided.

A NOTICE OF SITE VISIT WAS ISSUED AND MUST REMAIN POSTED FOR 30 CONSECUTIVE DAYS.
