25 | Licensing Program Analyst (LPA) Kate Huang conducted a Case Management inspection to follow up on an Unusual Incident that was reported by the facility to Licensing on 06/05/2025. LPA was greeted by Vanessa Paiz, director. LPA toured the facility, interviewed staff, and reviewed files. The teacher-to-child ratio was in compliance during today’s inspection.
Based on interviews and evidence gathered, it was determined that on 06/05/2025, between approximately 9:00 a.m. and 9:15 a.m., a toddler (C1) was left unattended in a room (or a large closet) within Classroom 166 for an estimated less than one to two minutes. The exact duration could not be confirmed, as statements from interviewees were inconsistent. At the time of the incident, the door to the room (or a large closet) was fully closed.
LPA observed that the room contained trash cleaning tools, including several long-handled brooms, a dustpan, a trash-picking grabber, multiple large bottles of white glue (Colle Blanche), and a large number of toys. The presence of these items in an unsupervised setting poses an immediate risk to the health, safety, and personal rights of children in care.
LPA informed director that this report dated on 06/13/2025 documents One Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA informed the director to provide a copy of this licensing report dated on 06/13/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview was conducted with the director, Vanessa Paiz. A notice of site visit has been issued and must remain posted for 30 days. Appeal Rights Given.
