Pertaining to the allegation, staff left day care child unsupervised all interviews conducted with staff determined that staff denied observing or having knowledge of any child being left unsupervised. All three (3) parents interviewed denied observing any child being left unsupervised.
In addition, on 11/13/25, 12/04/25, and 01/15/26, LPA Bahar conducted unannounced inspections at the facility and did not observe any child(ren) left unsupervised.
In regard to the allegation, staff negligence resulted in day care child sustaining an injury, all interviews conducted with staff determined that staff denied observing or having knowledge of any child sustaining an injury due to staff negligence while in care. All three (3) parents interviewed denied observing or having knowledge of any child sustaining an injury due to staff negligence while in care.
Based on the interviews conducted and records review, at this time, there is not enough evidence to support the above allegations. This agency has investigated the complaint, alleging that staff left day care child unsupervised and staff negligence resulted in day care child sustaining an injury.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegations are UNSUBSTANTIATED.
No deficiency was cited during today’s investigation.
Exit interview was conducted and report with reviewed with the Director, Han Lee. Appeal rights were provided.
A notice of site visit was given and must remain posted for 30 days.
