On 02/05/2026, Licensing Program Analyst (LPA), Syeda Bahar, conducted an unannounced complaint investigation to deliver the findings regarding allegations listed above. LPA met with the Director Han Lee and explained the purpose of today’s visit. During today’s investigation LPA toured the indoor and outdoor areas of the facility with the Director and a census was taken.There were 89 children observed to be present at the facility during this inspection.

Information provided by the Reporting Party (RP) alleges that staff left day care child unsupervised and staff negligence resulted in day care child sustaining an injury.

During the investigation, LPA Bahar obtained copies of facility roster (LIC9040) dated 11/14/25, 11/18/25, and 01/15/2026, personnel report (LIC500) dated, 12/4/25, 1/15/26 and other supporting documents. LPA also conducted five (5) staff interviews and reached out to Seven (7) day care parents, three (3) of which responded.

Pertaining to the allegation, staff left day care child unsupervised all interviews conducted with staff determined that staff denied observing or having knowledge of any child being left unsupervised. All three (3) parents interviewed denied observing any child being left unsupervised.

In addition, on 11/13/25, 12/04/25, and 01/15/26, LPA Bahar conducted unannounced inspections at the facility and did not observe any child(ren) left unsupervised.

In regard to the allegation, staff negligence resulted in day care child sustaining an injury, all interviews conducted with staff determined that staff denied observing or having knowledge of any child sustaining an injury due to staff negligence while in care. All three (3) parents interviewed denied observing or having knowledge of any child sustaining an injury due to staff negligence while in care.

Based on the interviews conducted and records review, at this time, there is not enough evidence to support the above allegations. This agency has investigated the complaint, alleging that staff left day care child unsupervised and staff negligence resulted in day care child sustaining an injury.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegations are UNSUBSTANTIATED.

No deficiency was cited during today’s investigation.

Exit interview was conducted and report with reviewed with the Director, Han Lee. Appeal rights were provided.

A notice of site visit was given and must remain posted for 30 days.

Based on interviews, Staff #1 disclosed placing Child #1 on a chair with a buckle (seat belts) due to Child #1 interrupting other children in the classroom. On 11/13/25, the Director stated that the facility has chairs with buckles, which are used when a child is unable to follow directions and is interfering with others by hitting or biting. On 01/15/26, a statement from the Director confirmed that Staff #1 placed Child #1 on a chair with a buckle (seat belts).

Interviews conducted with three (3) out of five (5) Staff disclosed that staff use a chair with a buckle (seat belts) to place children in the chair when they are disrupting other children in the classroom.

The police report, dated 11/05/25, confirms that the Director advised the officer that they have children’s chairs with seat belts to prevent children from moving around who have a hard time sitting still or tend to fall off their seat.

Based on the pictures dated 11/03/25 and video footage dated 11/04/25, obtained and reviewed by LPA Bahar, Child #1 was observed sitting on a chair with a buckle (seat belts) during classroom activities while at the day care.

Per the California Code of Regulation, Title 22, Division 12, Chapter 1, one (1) “Type A” deficiency has been cited (See attached LIC 9099-D). LPA Bahar informed Director, Han Lee that this report dated 02/05/26 document(s) one (1) Type A citation(s) which shall be posted for 30 consecutive days as there is immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Bahar informed the Director, Han Lee to provide a copy of this licensing report dated 02/05/26 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview was conducted and report with reviewed with the Director, Han Lee. Appeal rights were provided.

A notice of site visit was given and must remain posted for 30 days.

By the Plan of Correction (POC) due date, 2/6/26, all staff will renew the Mandated Reporter Training (MRT). All staff will take the Child Care Licensing Division (CCLD) video training on personal rights & supervision by 2/19/26. Online training video link was provided to the Director.

Based on interviews and records review, the licensee did not comply with the section cited above. Staff #1 restrained a day care child #1 by placing the child in a chair equipped with buckles (seatbelts), which poses a potential health, safety, and personal rights risk to the persons in care.

By 2/6/26, Director will register for the CCLD in-person orientation training & will submit the following to the Departments: New MRT Certificates of all Staff, Proof of Director’s CCLD Orientation in-person training registration, Video training agenda. By 2/19/26, Director will submit Attendance sheet, & written statement from all staff of their understanding of the regulations.
