All staff interviews indicated that staff denied observing or having knowledge of any child sustaining an injury due to staff negligence while in care. Similarly, all three (3) parents interviewed denied observing or having knowledge of any child sustaining an injury due to staff negligence while in care.
During interview of Child #1’s parent, it was disclosed that on 11/21/25, day care staff informed parent of the child #1’s injury and advised parent to seek urgent care. At the initial urgent care visit on 11/21/25, the physician noted unexplained injuries that could have resulted from physical contact. On 11/23/25, the parent of child #1 went to a different urgent care facility, where the diagnosis was reviewed: the injury was noted to be a possible insect bite or contact allergy to right forearm of the child #1.
Based on interviews conducted and records reviewed, there is insufficient evidence at this time to support the allegation. This agency has investigated the complaint alleging that staff negligence resulted in a day care child sustaining an injury.
Although the allegation may have occurred or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur. Therefore, the allegation is UNSUBSTANTIATED.
No deficiencies were cited during today’s investigation.
Exit interview was conducted, and the report was reviewed with Director Han Lee. Appeal rights were provided.
A notice of site visit was given and must remain posted for 30 days.
