On 06/17/2026, Licensing Program Analyst (LPA), Farida Raja conducted an unannounced complaint visit to deliver investigation findings for the above allegation. LPA met with staff, Manisha Kulshreshtha and explained the purpose of today's visit. Director, Madan Bellam arrived a few minutes later.

During today's inspection, LPA toured the facility and observed it to be operating within ratio and capacity requirements.

During the course of this investigation, LPA interviewed director, staff including former staff and parents and reviewed relevant records.

Based on interviews, 3 out of 12 individulas stated that the facility has been observed operating out of ratio when an unqualified staff (teacher aide) is left alone with children without a qualified teacher present both indoors and outdoors and when teachers step out of the classroom to obtain supplies from the kitchen.

LPA also discussed with Director the requirement for staff to step out of the classroom to permit entry to authorized representatives which could lead to the classroom being out of ratio.

Based on interviews and evidence gathered during the investigation, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED.

One Type B deficiency was cited as a result of today's inspection. Appeal rights provided.

Exit interview conducted and report was reviewed with Director, Madan Bellam.

A NOTICE OF SITE VISIT WAS ISSUED AND MUST REMAIN POSTED FOR 30 CONSECUTIVE DAYS.

Based on interviews, toddler and preschool classrooms were observed operating out of ratio when an unqualified staff is left alone with children both indoors and outdoors and when teachers step out of the classroom to obtain supplies from the kitchen. This poses a potential risk to the health, safety, and personal rights of children in care.
