25 | Licensing Program Analyst (LPA), Shine Yu, conducted an unannounced Annual Inspection and was greeted and met with the Licensee, Marina Gonzalez.
LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC610), Parents' Rights (PUB 393) and Earthquake Preparedness Checklist (LIC9148). Facility's operating days and hours are Monday to Friday 7:00 AM to 5:00 PM. LPA observed a current Children's roster. The last fire drill was conducted on 06/10/2025. Licensee acknowledged the fire drill has to be conduct once every 6 months. Present are licensee, 1 helper and 3 children (all are over 2 years old).
LPA reviewed 4 children's files and observed that notification of parent's rights forms, immunization records forms, consents for emergency medical treatment forms, and Identification forms are in each file.
LPA also reviewed the staff files for the following records: Employee Rights (LIC9052), Statement Acknowledging Requirement to report Child Abuse (LIC9108), and Immunization Record showing immunity to measles (MMR), pertussis (Tdap), and influenza. Licensee's pediatric 1st Aid and CPR certificate, expires 4/26/2026. Licensee's mandated reporter training certificate expired on 11/25/2026. The helper's mandated reporter training certificate expired on 10/28/2025. Licensee stated will have her to take the course immediately and will provide a copy of certificate to LPA when it's completed.
Licensee accompanied with LPA to tour the facility. The Off limits areas inside the home are all the bedrooms, the 2nd floor and garage. LPA observed the fire extinguisher (3A 40 BC) in the kitchen, the smoke/carbon monoxide detector is in the kitchen. The bathroom is clean and in working condition. The on limits areas inside the home are Dinning area, Den area, and living room. There are age-appropriate toys in the living room and play yard for children. Licensee stated there is no firearm in the facility.
